<p align="left"><font size="2">동맥경화는 염증성 질환이다. Hypercholesterolemia (고지혈증), 특히 LDL cholesterol이 높은 경우가 동맥경화의 주요한 위험 인자이므로 fatty streak (지방 선조)의 형성 과정은 지방의 축적 뿐 아니라 사실 더 많은 인자가 관여한다. 바로 fatty streak가 순수한 염증성 병변으로 동맥경화의 가장 초기 증상이라고 할 수 있다. Hypercholosterol로 인한 fatty streak를 형성하기 위해서 먼저 endothelial dysfuncton (내피세포 기능 장애)가 일어나는데, LDL의 증가 외에 흡연, 고혈압, 당뇨, 유전적 변이, 감염 인자 (Herpes virus, 클라미디아 균 등) 등이 원인이 될 수 있다. Endothelial dysfunction은 혈관 손상에 의한 대상성 반응으로 인해 endothelial cell (EC, 내피세포)의 정상적 항상성이 변화되어 생기는데, 혈관 손상으로 인해 혈관 투과성이 증가할 뿐 아니라, monocyte와 같은 leukocyte의 EC로의 유착이 증가한다. 그리고 intima 내에서 monocyte의 분화가 macrophage를 형성, LDL과의 결합으로 foam cell (거품 세포)을 형성한다. 지금부터 동맥경화의 초기 증상인 <strong>monocyte recruitment</strong>와 <strong>foam cell</strong> 형성에 대해 설명해 보겠다.&nbsp;<br />

① Monocyte recruitment</strong><br />
<br />
<strong>1. Adherenece of monocytes to arterial endothelium<br />
</strong>&nbsp;&nbsp; Monocyte 들이 활성화된 EC (endothelial cell) 에 발현된 cell adhesion molecule (P-, E- selectin ; rolling interaction 조절, VCAM-1, ICAM-1; firm adhesion에 관여) 에 의해 lesion&nbsp;&nbsp;&nbsp;<br />
prone site (병변 발생 부위)에 모이게 된다. (monocyte들은 integrin으로 EC에 고정)<br />
<strong>2. Penentration of monocytes into the artery<br />
</strong>&nbsp;&nbsp; 부착된 monocyte들이&nbsp;subendothelial&nbsp;region (intima)로 이동하는데, 여기에는 chemoattractant molecule들이 관여한다.&nbsp;<br />
&nbsp;&nbsp; -&nbsp;MCP-1 (monocyte chemoattractant protein-1)<br />
&nbsp;&nbsp; - CCR-2 (CC chemokine receptor-2)<br />
&nbsp;&nbsp; - CXCR-2 (CX chemokine receptor-2)<br />
&nbsp;&nbsp; - oxLDL ;&nbsp;산화된 LDL&nbsp;자체도 chemoattractnat로서, 이것의 산화된 phospholipid component들이 EC에 의한 MCP-1의 발현을 유도한다.<br />
<strong>3. Phenotypic modofication<br />
</strong>&nbsp;&nbsp; M-CSF (monocyte colony stimulating factor)와 같은 국소적으로 생산된 요소들에&nbsp;반응하여 monocyte가&nbsp;macrophage로 분화한다.<br />
<strong>4.&nbsp;SR expression<br />
&nbsp;&nbsp; </strong>SR-A (scavenger receptor A), CD36과 다른 oxLDL receptor들이 macrophage에서 발현<br />
<strong>5. Imbibition LDL &amp; make foam cell</strong><br />
&nbsp;&nbsp; LDL을 흡수해서 cholesterol ester를 포함하는 다수의 cytoplasmic droplet을 갖고 있는 foam cell을 형성한다.&nbsp;<br />

<strong><font color="#ff0000">**&nbsp;LDL은 산화, 당뇨병에서 당화, 응집, 면역 복합체로의 융합 등에 의해 변형될 수 있고, 이것이 내피세포 (EC)와 평활근세포 (VSMC) 를 손상시키는 주요한 원인이다. 그리고 동맥벽에 부착되어 축적, 산화 과정을 거치고 macrophage에 의해 함입되어 지질 과산화물을 형성하고, 촉직, foam cell을 형성하게 된다. LDL이 변형되어 macrophage가 이를 탐식하면, foam cell을 활성화시킨다. **</font></strong>&nbsp;&nbsp;&nbsp;<br />

<strong>② Foam cell 형성&nbsp;</strong><br />
1. LDL (low density lipoprotein) 입자들이 동맥벽을 통과하면,&nbsp;protepglycan이 LDL을 잡아서 산화에 대한 감수성을&nbsp;증가시킨다. 왜냐하면 LDL은 LDLR (LDL receptor)에 흡수 속도가 늦고, 그 receptor를 downregulation시키기 땨문이다. 반면 LDL의 변형된 형태인 acetyl LDL이나&nbsp;oxLDL은 이들의 receptor (acetyl LDLR =&nbsp;SR-A, oxLDLR = CD36, SR-A)&nbsp;에 대해 흡수 속도가 빠르고,&nbsp;receptor 또한 세포내 cholesterol의 증가와 반응하여 downregulation되지 않는다.<br />
&nbsp;&nbsp; -&nbsp;LDL의 산화에 관여하는 효소 :&nbsp;LO (lipoxygenase), MPO (myeloperoxidase), iNOS (inducible nitric oxide synthase), NADPHox (NADPH oxidase)&nbsp;<br />
2. VLDL&nbsp;(very low density lipoprotein)은 LPL에 의해 변형이 쉽게 일어나고, 그 결과 잔여 입자들은 proteoglycn에게 잡히고, 산화적 변형 (oxidativs modification)이 일어난 후, macrophage에게 흡수된다.<br />
3. Fatty streak로 인한 foam cell을 형성한다.<br />
&nbsp;&nbsp; - 축적된 산화된 lipid들은 fatty streak를 형성하고, 이들은 macrophage에 의해 흡수, macrophasge는 foam cell로의 변형이 일어나는 것이다.</font></p>
