</strong></font><font size="2">암의 전이는 일련의 복잡한 상황들이 암세포와 숙주 사이에서 지속적으로 일어나게 된다.&nbsp;지금부터 전이의 단계를 설명하겠다.<br />

</font><font size="2"><strong>1. Primary tumor (1차 종양)<br />
</strong>1) Tumor initiation&nbsp;<br />
&nbsp; - Oncogene에서의 돌연변이 또는 염색체의 재배열 (chromosome rearrangement)에 의해 primary malignant neoplasm (1차 악성 종양)을 형성한다.<br />
2) Tumor promotion &amp; progression<br />
&nbsp; -&nbsp;Endogenous,&nbsp;exogenous factor에 의한&nbsp;genome instability<br />
&nbsp; - Tumor cell에 대한 promoter 유전자는 활성화되고, suppressor 유전자는 억제됨<br />
&nbsp; - Clonal expansion<br />
&nbsp; - Loss of differentiation (분화능의 상실)<br />
&nbsp; - Apoptosis의 감소<br />

3) Tumor cell의 proliferation<br />
&nbsp;&nbsp;&nbsp;이 단계에서는 angiogenesis(혈관 신생)이 일어나는데, 일반적으로 tumor cell 내부는 hypoxia 상태가 되므로, 이를 극복, 즉 혈액 공급을 원할히 하기 위해 HIF-1 (hypoxia inducible factor - 1) 단백질의 발현을 증가시킨다. HIF-1은 tumor cell의 생존에 필요한 유전자를 turn-on시키는 전사 인자 역할을 한다. 다음으로 VEGF (vascular endothelial growth factor)의 발현 수치가 높아지고, 결국 tumor cell 쪽으로 새로운 혈관이 생성되는 것이다.&nbsp;&nbsp;&nbsp;<br />
<br />
<strong>2. Invasion of tissue</strong><br />
&nbsp;&nbsp; 이차종양의 시발과 진행이 뒤따르기 위해서는 정상 조직에서 침윤성 조직 (invasive tissue)으로의 전환이 이루어져야 한다.<br />
1) EMT (epithelial mesenchymal transition) : Invasive cancer growth에 중요한 것으로, epithelia로부터 mesenchymal cell들을 형성한다.<br />
2) 널리 퍼지기 위해서, soli tumor는 부착하려는 성질을 낮추어야 하는데, E-cadherin, integrin과 같은 cell adhesion molecule의 발현을 억제한다.<br />
3) 원래의 조직으로부터 ECM (extra cellular matrix)을 침투하기 위해, MMP와 같은 여러 proteolytic enzyme들을 생산하면서 침투, 이동한다.<br />

</font><font size="2"><strong>3. Intravasation<br />
&nbsp;&nbsp; </strong>혈관계로 tumor cell이 들어가는데, 암세포는 lymphatics (임파관), venules (작은 정맥), capilaries (모세혈관)으로 들어간다. 단계는 vascular basement membrane (기저막)을 관통, interstitial stroma (간질 기질)로의 침임, 다시 기저막 관통 순으로 진행된다.<br />

<strong>4. Circulation in lymph and blood</strong><br />
&nbsp;&nbsp; Immune surveilance로부터 회피, mechanical stress에 대한 저항, platelet에 부착, 각각의 서로 떨어져 있는 organ의 endothelium에 부착 등의 기작을 통하여 이루어진다.<br />

</font><font size="2"><strong>5. Extravasation<br />
&nbsp;&nbsp;</strong>이 단계에서는 body 내의 어느 곳에서든지 tumor cell이 4단계인 circulation으로부터 나온다.<br />
1) &nbsp;Endothelial cell 상에서 tumor cell이 embolus 형성하면서 부착<br />
2) &nbsp;Endothelium의 수축<br />
3)&nbsp; Basement membrane에부착<br />
4) &nbsp;Basement membrane의 용해<br />
5) &nbsp;Tumor cell의 이동<br />
<br />
</font><font size="2"><strong>6. Metastatic tumor<br />
</strong>&nbsp;&nbsp; Secondary tumor colony의 형성을 원할히 하기 위해 host의 tissue와 microenvironment를 형성한다. 여기에는 local tissue growth factor의 receptor, adhesion site, angiogenesis factor 등이 관여하고 metastasis suppressor gene에서의 돌연변이도 포함된다.<br />
